These are the client forms needed for the initial intake. You may either fill them out electronically. You may also print the forms and fill them out by hand.
Instructions for completing them electronically:
1. Download and save the form to your device.
2. Type directly onto the form.
3. Submit the form by printing it and dropping it off at the office, emailing the electronic copy to whitney@daugettefamilyhealth.com, or uploading the documents directly into your portal.
